MAKING PLANS WITH HOSPICE IN HIS CARE:

THREE EASY STEPS

Referrals and requests for more information about hospice may come from patients, families, friends, healthcare professionals, and spiritual counselors. There is an easy three step process to make a referral to Hospice in His Care or simply schedule an information-only visit.

Step One:

Place a call to Hospice in His Care at (225) 214-0010 24 hours a day, 7 days a week for information about the benefits of hospice or a request for hospice services.

Step Two:

Our Information Facilitator will contact all responsible parties to coordinate family consultations and/or patient assessments.

​

Step Three:

Our Admit Team Member will consult, evaluate and initiate care, as deemed appropriate by a physician.

A Hospice in His Care representative is available to speak to your organization about Hospice.
